Guidelines:

All payments and documents
must be submitted and received by Prudentialife
from May 1, 2008 to July 31, 2008 to
qualify for this Reinstatement Drive. |

For pended reinstatements, re-submission
of Reinstatement Form is required for
the plan to be qualified under this
reinstatement drive. All installments
in arrears and/or penalties must also
be fully settled. Please note that any
payment already made for Reinstatement
Fee and penalties prior to the promo
period will not be subject for refund. |

To authenticate the signature of the
Planholder, a photocopy of one (1) valid
ID is required for purposes of this
Reinstatement Drive. |
