Dates will be based on the Contract Date of
the plans purchased. Returned Plan Applications
(RPA) must be rectified within the Cut-off Date,
based on the Contract Price of the plan.

- Cut-off dates will be based on a payment’s
respective Official Receipt (O.R.) date
