Prudentialife continues to operate as a servicing pre-need company and pay plan benefits of our planholders as warranted.

We also continue to market our other services that include Health Plans, Mutual Funds, and Insurance products among others.

For pre-need Planholders, you may wait for your plan to mature and thereafter claim the maturity values/benefits due to you. If you wish, you can choose to terminate your plan now and claim its present value.

Plan Termination requirements:

	1. Original Plan Contract / Contract Policy
	2. Original Certificate of Full Payment
	3. Photocopy of at least 2 valid IDs of Planholder (passport, driver's license, SSS ID, etc.); Planholder must also affix his signature thrice on the photocopy of his IDs.
	4. Benefits Settlement Application Form (3 copies), all copies must be accomplished in original ink) - Download Here
	5. Processing fee of Php 300.00 (to be deducted from the proceeds of Plan Termination)

Important Reminders:

	1. If planholder will claim his check personally, he only needs to present two valid IDs and sign on the release waiver portion of the plan termination application.
	2. If check will be claimed by a representative, representative must present planholder's notarized release waiver and quit claim,authorization letter, and photocopy of planholder's and representative's ID.
	3. If Planholder is deceased: photocopy of planholder's death certificate with registry number; marriage contract, birth certificate of planholder's children who are beneficiaries
